Step 1: Fill Out Your Information

Parent information: Have your work phone, cell phone, and email addresses on hand.
Student details: You will need your child’s health and medication information.
Emergency contacts: Please have the names, addresses, and phone numbers for your emergency contacts ready.
Start the application: Click the Online Registration link to begin.
Enter your info: Follow the simple prompts to type in your family and student details.
Update your records: Use this tool to enter your address and the name of the person to call in an emergency.
Save for later: If you need to stop, you can save your work and finish it later.
Submit: When you are finished, send the information to the district. You can also print or save a summary of your form for your records.

Step 2: District Review and Approval

District Review: Once you submit your application, the school district will review it.
Approval Status: The district can approve your request, put it on hold if they need more info, or deny it.
