Screening Program

Lyons School District 103 will have openings in the Spring for the Preschool For All Programs. Students who qualify after screening may be invited to join a class.  

All preschoolers who are 3 to 5 years old are eligible for screening. The screening will include an evaluation of several areas important to child growth, development, and success in school. Some of the areas include vision, hearing, speech, and language, cognitive, and social skills.

8:30am – 11:00am 

Please fill out a screening request form (Preschool Screening Consent Form) or call the Preschool Office at 708-783-4100 ext. 1016 to make a screening appointment. 

Please bring: your child, your ID, your child’s birth certificate, updated physical exam (if you have it), and proof of income (2 most recent pay stubs or your W2 forms).

It is very important that every eligible child and parent take advantage of this opportunity provided by the district. There is no fee for screening. The screening will take place at Edison School, 4100 Scoville Ave. in Stickney IL.

ALSO AVAILABLE - Birth to 3 Screening: The Ages & Stages Questionnaire is available for those children Birth-3 years of age. This involves the completion of a parent questionnaire and a brief interview with one of our Parents as Teachers staff members.
